New state in the Plone issue tracker: fixed+released

At the prompting of Wichert, we have introduced a new state "fixed+released" in the issue tracker at http://dev.plone.org/plone.

The reasoning is that sometimes it's hard to know whether a fix was included in a release yet, or is just fixed in SVN. This makes it possible to track these things for the release manager and others.

You can still close tickets by including the standard "fixes #1234" in your commit message.

This could of course be improved by not counting "fixed" tickets as closed, but I assume that needs some more discussion (and I'm not sure it would be used by anyone).
