Insert effects (generate) should now set the LEN global

To reduce memory consumption during effect processing, a recent change
has been made that affects the displayed progress dialog.  With this
change, Audacity should now be told how much audio is going to be
generated by storing the number of samples in the LEN global variable.  
If the variable is not set, then the progress dialog will only show
elapsed time, with no estimate of remaining time.  Again, this only
affect the INSERT effect type.

Good news is that memory requirements have been greatly reduced.
