It seems someone--likely to be me--broke the CBR presets in LAME 3.98.
Preset insane is mapped to preset 320 (ABR), but help says it should be CBR.
Preset 320 can not get a CBR preset by adding cbr keyword, but help says it
should be possible.
Needs to be fixed before 3.98 stable release.
